HomeServe USA Repair Management (Florida) Corp. (“HomeServe”), license number P129179, with corporate offices located at 601 Merritt 7, 6th Floor, Norwalk, CT 06851, is an independent company separate from Sunshine Water Services and offers this optional service plan as an authorized representative of the contract issuer, Northcoast Warranty Services, 800 Superior Avenue E, 21st Floor, Cleveland, OH 44114. Your choice of whether to purchase this service plan will not affect the price, availability or terms of service from Sunshine Water Services. Sunshine Water Services and HomeServe entered into an agreement to introduce these plans to Sunshine Water Services customers.

Disclosure/Rebate Schedule: HomeServe USA Repair Management (Florida) Corp. (HSRMF) will offer the purchasers of the following home service contracts (sold in connection with HSRMF's relationship with Utilities Inc.) an instant rebate for the products listed below. These contracts are provided by Northcoast Warranty Services, Inc.. The amount of and the availability of the rebate will by no means depend upon the age, sex, place of residence, race, nationality, ethnic origin, marital status, or occupation of the consumer. The rebate will be available to all customers purchasing these coverages.

Water Service Line Coverage: For the Water Service Line Plan, which normally costs $5.99, HSRMF will provide an instant rebate of $3.00 per month for one year, or 50% of the cost of the plan. This means that you will pay $2.99 per month for one year for the coverage after the rebate.  

Water Service Line Plus Coverage: For the Water Service Line Plus Plan, which normally costs $7.98, HSRMF will provide an instant rebate of $3.99 per month for one year, or 50% of the cost of the plan. This means that you will pay $3.99 per month for one year for the coverage after the rebate.  

Sewer Septic Line Coverage: For the Service Septic Line Plan, which normally costs $10.99, HSRMF will provide an instant rebate of $5.50 per month for one year, or 50% of the cost of the plan. This means that you will pay $5.49 per month for one year for the coverage after the rebate.  

Exterior Service Line Plus Coverage: For the Exterior Service Line Plus Coverage, which normally costs $18.97, HSRMF will provide an instant rebate of $9.48 per month for one year, or 50% of the cost of the plan. This means that you will pay $9.48 per month for one year for the coverage after the rebate.

Internal Plumbing & Drainage Coverage: For the Internal Plumbing & Drainage Plan, which normally costs $11.99, HSRMF will provide an instant rebate of $6.00 per month for one year, or 50% of the cost of the plan. This means that you will pay $5.99 per month for one year for the coverage after the rebate.
